Paperwork:
All paperwork must be turned in either by email, mail or dropped off to Maple Ski Ridge, at least THREE WEEKS (21 days) prior to your child attending Camp Maple. Paperwork includes a REQUIRED NYS SCHOOL HEALTH EXAMINATION FORM and an immunization record that is less than 1 year old from your child’s doctor. A valid school physical will suffice.
Medications at Maple Ski Ridge
Our Assistant Health Directors will have CPR, First Aid, and EPIPen Certifications, but do not have the credentials to administer medication (other than emergency EPIPens).
If your child requires any daily medication administered during camp hours- they will have to be able to 100% self administer without ANY assistance other than being watched OR parents will have to come to camp to administer the medications themselves. This includes any OTC medications, antibiotics, and rescue inhalers.
CPR & 1st Aid trained staff will have an EPIPen Certification and will be able to ASSIST your child in self-administering this medication. This would mean wrapping their hand around your child's hand to assist them in administering this life saving medication in the event of an allergic reaction.

A copy of this completed form needs to be submitted to our Health Director no later than 3 weeks before child's camp start date. Must be dated less than a year from camp start date.
MEDICATION CONSENT FORM
If your child needs self administered medication while at Camp Maple, please fill out and return this form to our Health Director.
