We use the NYS school physical form for our medical form. Your doctor's office may have this form on file or you can download a copy below. Also include a copy of their immunization records. All children attending camp must be vaccinated as per NYS school requirements.

 

You may use an existing school physical from your doctor's office if it was done within 12 months. of your child's registered camp dates.

Completed health forms and immunization records must be uploaded directly into camper registration no later than 14 days before the first date of camper attendance.
